PHYSICAL THERAPIST
JOB DESCRIPTION
JOB SUMMARY:
The Physical Therapist will complete physical therapy evaluations for all patients, as prescribed. The Physical Therapist will complete a written report of findings within one week of completion of testing and will submit this report to the appropriate clinic designee. The Physical Therapist will also develop individualized physical therapy treatment plans and conduct therapy sessions, as indicated. The Physical Therapist will assist with completion of other documentation related to physical therapy evaluations and therapy, as appropriate.

ESSENTIAL JOB FUNCTIONS:
1. Completes physical therapy evaluations for all new patients as prescribed/indicated. Re-evaluations for all current patients will be completed every six months for patients younger than two years of age and annually for all patients aged two years and above. In addition, all patients discharged from physical therapy services will be re-evaluated six months following discharge to assess for possible regression.
2. Completes a formal written report of findings and submits to the appropriate clinic designee.
3. If the patient qualifies for physical therapy services, the Physical Therapist will develop an individualized physical therapy treatment plan.
4. Reviews and updates each patient’s treatment plan on a quarterly basis. The Physical Therapist will document these reviews and update the current Individualized Treatment Plan (ITP) located in the patient’s medical chart.
5. Conducts physical therapy services, as appropriate. The number and length of these sessions will be determined by the Physical Therapist in accordance with Kids for the Future Therapy Group, Inc. and Arkansas Medicaid’s therapy guidelines.
6. For every service billed, the Physical Therapist will provide progress notes in the appropriate format which contains all information as required by Kids for the Future Therapy Group, Inc. policy, EIDT guidelines, and/or other applicable regulations. The Physical Therapist will keep detailed records of all work with all patients he/she serves.
7. Assists with completion of other documentation related to physical therapy evaluations and therapy, as appropriate.
8. Refers patients for additional clinical services, as needed.
9. Attends and actively participates in job-related center functions such as parent meetings, transition conferences, peer reviews, conferences with supervisors, staff workshops, and job orientations, as indicated.
10. Represents the program to parents, visitors, and the public in a positive way.
11. Maintains familiarity with health and safety guidelines, including universal precautions and daily operating procedures, as implemented by the clinic nurse.
12. Immediately reports any information regarding health and safety concerns to supervisor or clinic nurse.
13. Completes relevant and approved continuing professional education each year as required to maintain professional licensure.
14. Is familiar with the Kids for the Future Therapy Group policies and procedures and guidelines, as well as those requirements set forth by regulatory agencies, including Arkansas Medicaid, EIDT, PASSEs, DPSQA, OT / PT / SLP Regulations, AR Child Care Licensing, and the AR Health Department.
15. Displays concern for the rights of patients and families served by the clinic.
16. Reports for work dressed appropriately and complies with established company policies regarding dress code.
17. Ensures confidentiality of patient and employee information and responds to internal and external information according to clinic policy.
18. May perform other related and non-related duties, as assigned and agreed upon by both parties.
19. Regular and punctual attendance is required.
20. Has the ability to work required overtime.
QUALIFICATIONS:
1. A minimum of a Bachelor’s degree in Physical Therapy from an accredited college or university.
2. A current Arkansas license as a Physical Therapist.
3. Membership in the American Physical Therapy Association.
4. Prefer at least one year’s experience working with infants/toddlers and/or preschool children with developmental disabilities and/or special needs.
ENVIRONMENTAL CONDITIONS:
1. Must be able to lift up to 50 pounds.
2. Must be physically able to walk, stand, and stoop for several hours each day.
3. Must be willing and able to sit on the floor and get up without assistance.

JOB STATUS: This position is a full-time, hourly position. This is a safety sensitive position.
